Real-world efficacy and side effect profile of bempedoic acid in a tertiary centre lipid clinic

Abstract
Introduction: Bempedoic acid, an ATP citrate lyase inhibitor approved by NICE in April 2021, is an alternative treatment for patients intolerant to statin or other lipid-lowering therapy with high atherosclerotic risk. Furthermore, it can be used in combination with other lipid-lowering medications to reach low-density lipoprotein cholesterol (LDL-c) targets. The utility of bempedoic acid is supported by randomised controlled trial data, but its real-world efficacy and side-effect profile remains largely unexplored. This study seeks to investigate early outcomes of bempedoic acid use in a tertiary centre lipid clinic. Methods: We performed a retrospective analysis of electronic healthcare records of 100 patients attending our lipid clinic who were prescribed bempedoic acid (Nilemdo or Nustendi) between 1 st May 2021 and 8 th December 2023. Lipid profiles and other biochemical parameters were collected prior to treatment initiation and repeated within one year follow-up as part of routine clinical care. Results: Of the 100 patients, 86 were identified as intolerant to one or more lipid-lowering medications. 22 patients had experienced an atherosclerotic event prior to starting bempedoic acid. 34 patients reported side-effects on bempedoic acid, with 21 patients stopping the medication as a result. Myalgia was the most commonly reported sideeffect, with 15 patients affected, and 8 of these discontinuing the medication. Other side-effects included joint pain, diarrhoea and headache. Amongst patients who continued bempedoic acid, mean baseline total cholesterol decreased from 5.36 ± 1.26 mmol/L to 4.47 ± 0.97 mmol/L (p<0.0001) and mean baseline LDL-c decreased from 3.20 ± 1.03 mmol/L to 2.37 ± 0.85 mmol/L (p<0.0001). Across 20 patients with prior ASCVD events who continued bempedoic acid treatment, mean baseline LDL-c decreased from 2.96 ± 1.08 mmol/L to 2.27 ± 0.91 mmol/L (p<0.01), although none of these patients achieved their target LDL-c of 1.4 mmol/L. Conclusion: In this real-world study, treatment with bempedoic acid led to a significant decrease in LDL-c, but there was a relatively high incidence of side-effects reported, particularly myalgia.
